China Crew Visa Requirements
Collect and send the following documents:
1. Valid U.S. Passport
You must send your valid, signed U.S. passport. Make sure your passport is still valid for at least 6 months, and has at least two blank visa pages. If not, you need to renew your passport first.
Foreign passport holders: In addition you must send a copy of your valid Green Card or U.S. Long Term Visa, Holders of U.S. short term tourist/business visas cannot apply in the USA.
2. Passport Photograph
You must provide one recent passport photograph. photo must be in color with a plain light or white background and full front view of face.
3. Visa Application Form
Click here to complete the visa application form online. Make sure all fields are fully completed. If a field does not apply to you, mark it "N/A" - do not leave it blank. The completed form should be clearly printed out, and signed by the applicant.
4. Company Letter
A letter from the relevant company (i.e. airline/transport company) on the company letterhead addressed to the china consulate, It should contain the applicants name, position, and request for a visa.
5. Work ID
A copy of your work ID/Crew Badge.
6. Fees
Consular Fee
Make a Money Order payable to the "Chinese Consulate General." Do not send personal or company checks. This fee may also be paid together with our service fee to "Deluxe Passport Express"; or by credit card, by entering your credit card information on the service order form (see below).
